Guidelines

Membership fee is due in full the day you join.
Members can sign up by calling or visiting our office.

The discounted fee for services rendered are due the day of service.

The membership expires 12 months (365 days) from join date.

Missed appointments may not be rescheduled at our discretion. Any unused benefits will be forfeited and do not roll over.
